A 15 Minute Follow-up Nutritional Consultation with our Functional Medicine Doctor. Please call our office to set up a time for your consultation.
This consultation appointment is most commonly made for established patients with problem-focused medical health concerns and history and straightforward medical decision-making.
This request requires the interested person to become a new patient at our office. If you cannot visit our office, we are happy to meet with you for this consultation through either teleconference or by telephone.
We will need to initially email to you a patient follow-up present health status form in advance to fill out and ask you to return them completed to us by either regular mail, by fax, or to scan the completed forms into your computer to email as attachments to us. After we receive the completed forms we will confirm your appointment with our Doctor.
